Bisoprolol is a beta-1 selective blocker without its own sympathomimetic activity, it does not have membrane-stabilizing action.

The medicinal product produces an antihypertensive, antiarrhythmic, and antianginal action.

When it is used at medium therapeutic doses, compared to non-selective beta-blockers, the medicinal product exerts less prominent effect on the organs containing beta₂-adrenergic receptors (pancreas, skeletal muscles, smooth muscles of peripheral arteries, bronchi, and uterine) and on carbohydrate metabolism; it does not lead to retention of sodium ions within the body; the degree of atherogenic effect does not differ from that of propranolol.
